Format of each ticket is the same (except the 1944 ticket has "1944" underprinting the body text presumably as some security feature) and features an oval photo portrait of President Roosevelt.

Body text on 1942 ticket (same in other years except updated to sixty-first and sixty-second respectively):
"This Birthday Ball in honor of President Franklin D[elano]. Roosevelt is on of may similar affairs being held in the cities and towns throughout the United States on the occasion of his sixtieth[sixty-second birthday.
Fifty percent of the net proceeds of the Ball is to be used for the rehabilation of Infantile Paralysis [polio] sufferers; and fifty percent of the net proceeds of the Ball will be turned over to the Committee for the Celebration of the President's Birthday for the National Foundation for Infantile Paralysis, Inc., for the continuation of its part in the national fight against Infantile Paralysis."
