No date, 1915 to circa 1930. Printed form, 4" x 9" wide, press numbered 1253. The Commission form of government took effect in 1915 and the late date is estimated by the twenty-five dollar fee which probably went up over time. The jitney bus or omnibus was a popular type of transport starting in the mid-1910s and continued into the 1920s.
