Order form pad for Keystone Dairy Co. (Hoboken) for home delivery; to be used by a woman (housewife), no date, circa 1916-1925. Printed form, 2-7/8" high x 4-1/2" wide, bluish-green paper with dark blue ink; 109 sheets padded with adhesive at left edge.
While the company's address is not stated, Keystone was based in Hoboken where it had its processing and distribution.
