Clear glass vial, 11/16" diameter x 1-7/16" high with clear molded plastic stopper with maroon plastic round insert on top with raised script "Ayerst"; printed 2" diameter round paper label trimmed top and bottom to fit on bottle; ink-stamped and holographic ink entries. Original contents present of two white tablets, type of medicine unknown. Inkstamped with the prescription number and the date which is lacking the last digit as it ran onto the glass - probably late 1950s. Written text includes instructions (one every 4 hours 4 times a day) and doctor's name (Dr. Abrams.) The label is the same format as a complete one found on objects 2011.005.0005 where it lists two pharmacists at the top: L.C. Schmidt; M.L. Kusseluk.
