Printed form with holographic carbon entries. Dated August 15, 1901. Person injured was William Gallagher on track 49, Export Yard, Hoboken, on August 14, 1901. No witnesses to the accident. He was a brakeman. The crew members of the train on which he was injured are listed with their addresses of which three were from Hoboken. Nature and extent of injury: "Testicles injured slightly."
