Single sheet printed one side, 10-1/2" wide x 8" high; folded to 5-1/4" wide x 4" high (printed fold instructions.) All but 3 of 24 coupons used; paper is not security paper or watermarked. (Coupons are torn out; creased for tearing, but not perforated.) Issued in World War II by the Office of Price Administration, a U.S. Government agency (OPA Form R-302.) Back has Becker's signature. The interior is the Certificate of Registrar which is a form that records full name and address as well as height, weight, eye color, hair color, age and sex. It is signed by registrar, Elsie V. Cox. Ration coupons were also called 'stamps.'
