Single plate, no page or plate number, 12-9/16" high x 8-15/16" high; horizontal crease at center. Not stated but the architect was H. Edwards-Fickens, 19 West 22nd St., N.Y. (His name is more commonly indexed as H. Edward Fickens without the hyphen as seen on the plate.)
