Note that the only product pre-printed on the form is 1cent [cent symbol] Chocolate Tootsie Rolls. See archives 2011.005.0036 for several invoices from 1937 when the company was still in New York City and the invoices were pre-printed with five products.
The Good Housekeeping seal states 'Institute' here whereas the ones from 1937 states 'Bureau.'
They were apparently continuous multi-part forms with interleaved carbon paper; the ones present being an original and thus the top sheet or copy 1 (unstated.) Fine perforations are found along top and bottom with pin feed holes at the sides.
