Cocomalt premium: Buffalo Bill Puzzle. Issued by R.B. Davis Co., Hoboken, N.J., 1933. Printed envelope, 7" x 10" high; 65 piece die-cut cardboard puzzle pieces (assembled: 6-1/2" wide x 10" high); with enclosure: Flight Commander Test pamphlet, single folio, 5-1/2" wide x 8-1/2" high, [4] pp.
All items state Hoboken, N.J. which was where Davis was based at the time and made the Cocomalt product. A one pound lithographed metal can of Cocomalt is depicted on the envelope back. Envelope and puzzle copyright 1933; pamphlet 1932.
The Flight Commander pamphlet has some entries on the page [4] application form. Ten-year old Ann Robison of Gansevoort, New York entered her name, address, age and weight (62 ilbs.) and then on June 6, 1933 entered her weight after a week of drinking Cocomalt. It ws 63 pounds.

On envelope back is the usual promotional copy on the product and "How to Become a Flight Commander" (on flap at bottom.) Also, it promotes the educational aspect with a "How to Use the Puzzle" section. It has instructions plus a "Time Chart." It gives 2 completion times, either "Good" or "Excellent", for ages 8, 12, 15 and adult
