No date, circa 1935-1940. Printed cardstock folder, 3-1/2" wide x 4-1/4" high x 1/8" thick with one blank score sheet. With: two padded blank score sheets laid-in (same format but a lighter, different paper than one in folder.)
Cover depicts the Teaneck building, "Colonial Home for Service." Volk was a long-time Hoboken funeral home; the Teaneck operation was founded in 1935. Addresses for both are on the back cover. It is likely this score pad with scoring rules was a promotional advertising item primarily for the added location.
